• Copy of the certificate of registration (Form 4-OPP).

• Copy of the VAT payer registration certificate.

• Copy of the registration certificate with the Pension Fund.

• Copies of registration certificates with social insurance funds.

• List of objects where work is planned to be performed.

*The selection of works is made in accordance with the List of high-risk works, which is provided by the legal company upon request.
